Chronic diseases require sustained medication use and daily self-management, yet adherence remains difficult because of complex regimens, limited health literacy, fragmented follow-up, and inadequate behavioral support. This study evaluated a nurse-led digital intervention integrating mobile medication reminders, individualized education, symptom and vital-sign monitoring, self-management goal setting, and asynchronous nurse feedback for adults with chronic diseases. A quasi-experimental pretest-posttest design with a comparison group was modeled using a simulated dataset of 240 adults receiving primary healthcare for hypertension, type 2 diabetes, or chronic heart failure. Participants were allocated equally to a twelve-week digital nursing program or usual care. Outcomes included medication adherence, self-management capacity, self-efficacy, digital engagement, and selected clinical indicators. The intervention group demonstrated greater improvement in medication adherence scores, self-management behavior, and self-efficacy than the comparison group. Participants with higher engagement in nurse messaging and self-monitoring achieved the largest gains, while older age and lower baseline digital literacy were associated with slower uptake. Improvements were also observed in systolic blood pressure and glycemic indicators among relevant diagnostic subgroups. The findings indicate that digital technology is most useful when embedded in relational nursing practice rather than delivered as automated reminders alone. Personalized nurse feedback, simple interfaces, and structured monitoring may strengthen adherence and patient capability. The study contributes an integrated nursing framework for scalable chronic disease management while emphasizing digital inclusion, privacy, clinical escalation protocols, and continued evaluation before routine implementation These findings support patient-centered digital nursing practice across chronic
