Chronic disease management requires sustained medication adherence, patient empowerment, and improved quality of life. Nurse-led digital interventions can extend therapeutic communication through mobile applications, telemonitoring, remote education, and virtual consultations. This article examines evidence on digital nursing interventions and proposes a conceptual relationship between medication adherence, empowerment, and quality of life. A directed literature review and narrative synthesis were undertaken, emphasizing peer-reviewed studies published between 2022 and 2025. Evidence suggests that digital tools combined with active nursing support can strengthen self-monitoring, treatment understanding, and patient engagement. Nevertheless, quality-of-life outcomes vary according to disease characteristics, digital literacy, intervention intensity, and follow-up duration. Patient empowerment may support adherence, while quality of life should be evaluated as a distinct multidimensional outcome. A staged evaluation framework is proposed, integrating technology access, nurse-patient communication, adherence, empowerment, and health-related quality of life. These relationships represent evidence-informed propositions rather than findings from a new clinical trial. Future studies in Indonesia should evaluate effectiveness using validated instruments, appropriate comparison groups, data privacy safeguards, and sufficient follow-up. Equity of access, family support, and individualized care remain essential considerations for implementation.
