Continuity of nursing care is essential for patient recovery across hospital, primary care, and home settings. This integrative literature review examines digital communication, self-management support, and therapeutic relationships using research published between 2022 and 2025. The synthesis indicates that structured digital follow-up can improve information exchange and symptom monitoring, while individualized education and collaborative goal setting strengthen self-management. Empathy, trust, and responsive nursing communication remain indispensable in remote encounters. Implementation requires equitable digital access, data protection, interoperable records, clinical escalation protocols, and ongoing evaluation. The proposed framework is conceptual and requires prospective validation before effectiveness can be established in Indonesian health services.
