Abstract

Primary healthcare requires innovations capable of expanding education, strengthening disease prevention, and ensuring the continuity of care at the community level. Digital interventions offer tools for communication, monitoring, and service reminders; however, their success is contingent upon health literacy, social support, access to technology, and the readiness of facilities. This article aims to develop a conceptual framework for strengthening primary healthcare through the integration of community-based digital interventions, health literacy, preventive behaviors, and the sustainability of health outcomes. A structured narrative literature review was conducted, synthesizing themes related to user factors, intervention design, the involvement of community health workers, accessibility, and evaluation. The synthesis indicates that digital education must be complemented by human support, easily understandable materials, alternative service channels, and data protection. Health literacy serves as a foundational capacity that influences an individual's ability to evaluate information and implement preventive measures. Community health workers and families act as bridges between digital messaging and daily behaviors, while primary healthcare centers (puskesmas) ensure clinical follow-up. Sustainability is assessed through consistent service utilization, regular screening, adherence to follow-up care, and the maintenance of healthy behaviors beyond the active intervention phase. The article proposes a phased implementation model, encompassing needs assessment, co-design with the community, integrated execution, and longitudinal evaluation. This model is conceptual and has not yet been empirically tested. The contribution of this study lies in integrating the dimensions of technology, community empowerment, and primary care governance to promote equitable benefits and strengthen disease prevention. Further research is required to determine effectiveness, costs, user acceptance, and the equity of outcomes across diverse population groups. These findings provide an initial foundation for developing programs that are adaptive to local contexts.
